Support and info for patients and carers affected by long term health conditions
Our support hub was set up in March 2018 to provide help and information to people with long term health conditions and their carers. The support hub is here to help reduce the stress in times of already great worry and uncertainty.
The support hub consists of:
- The welfare benefit advice service – at the core of the support hub, our team of welfare benefit advisers support patients with an array of issues including benefits (e.g. applications and appeals) and housing issues. We help people to navigate complex situations when going through difficult times.
- Volunteer ‘check in and chat’ – this service was set up originally during COVID-19 to support people who are more isolated than usual. We connect people with a volunteer buddy to regularly check in with them.
- Support groups – we run an amputee support group; this continued throughout COVID-19, running virtually.
- General information and signposting – the support hub team can also advise and signpost to many other services in the community, locally and nationwide dependent on patient or carer need.
